Who Can Request a Certified Copy

You must have a direct and tangible interest in the record. Entitled people include:

• The person named on the record
• Spouse
• Parent
• Grandparent
• Child
• Grandchild
• Brother or sister
• Legal guardian
• Legal representative

Legal guardians and legal representatives must provide additional proof of guardianship or representation.

Applicants must be at least 18 years old.

Identification Requirements

Applicants must provide a current government-issued photo ID or driver's license.

If you do not have a valid photo ID, alternative forms of identification may be accepted.
